As a Case Manager, you will be a vital member of our interdisciplinary team, working closely with patients, families, and healthcare professionals to ensure the best possible outcomes.

Key Responsibilities:
--- Completes departmental orientation, initial and annual competencies
--- Collaborates with other departmental staff to identify barriers to care and develop solutions
--- Completes documentation per workflow timeline and content requirements
--- Schedules family conferences and communicates with caregivers to keep them informed of progress
--- Coordinates weekly patient care team conferences to facilitate development, monitoring, and refinement of treatment plans

Qualifications:
--- Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist
--- Certification in Case Management or Rehabilitation Nursing preferred
--- Minimum of 2 years social work or case management experience in an inpatient setting highly preferred
--- Effective oral and written communication skills in English, additional languages preferred
