We are seeking a skilled and compassionate Patient Care Manager and RN Hybrid to join our team at Caretenders Home Health. As a key member of our clinical team, you will be responsible for coordinating and supervising clinical services, ensuring high-quality patient care and outcomes.Key Responsibilities:

• Supervision and Coordination: Supervise and coordinate clinical services to ensure delivery of quality patient care and outcomes.
• Clinical Services: Provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
• Patient Assessment: Continuously assess patient needs and ensure care rendered is individualized to meet those needs, appropriate, and reasonable.
• Coordination of Care: Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.Requirements:
  • Current RN licensure in state of practice
  • Current CPR certification required
