Job Description:
A company is looking for a bilingual remote Certified Medical
Assistant to work with chronically ill patients in Chronic Care
Management services.
Key Responsibilities:
Conduct monthly phone calls with patients and healthcare teams to
ensure care needs are met
Create and revise personalized care plans in coordination with
healthcare providers
Identify social determinants of health and coordinate resources to
address patient needs
Required Qualifications:
At least 1 year of experience as a Certified or Registered Medical
Assistant
Proficiency in both English and Spanish, including reading,
writing, and speaking
Experience with multiple EMR/EHR systems and Microsoft Office or
Google Suite
Ability to work on a Pacific Time Zone or Mountain Time Zone
schedule
Willingness to complete 6 weeks of training in Central Time
Zone
